6. A preliminary phone screening is conducted to ensure candidacy for LASIK surgery.
7. The Recipient selects a local provider from the list of local credentialed, Board Certified Ophthalmologists.
8. If for any reason the Recipient chooses not to have LASIK surgery, or is not a candidate, the gift amount purchased is fully refundable.
